Fill in a questionnaire and enter our prize draw!

We are currently carrying out a survey of how well we communicate with our patients. This survey was writen by the Barnet, Enfield and Haringay (BEH) Primary Care Trust (PCT). If you have been given a green raffle ticket you can use it to log in to our online survey. When the survey is finished all those who took part will be entered into a prize draw, prizes to be confirmed but likely to be shopping vouchers. As the survey has to be anonymous we will not know who has which number so please check this website or see the notice board in the surgery to find out if you have won.

Some patients will have filled out the survey on paper in the surgery, they will also be entered into the same prize draw. If you have already filled out the survey without getting a raffle ticket please ask at the front desk if you would like to take part.
